At Hometown Diagnostics, your child will be assessed through academic and cognitive testing to find patterns of strengths and weaknesses that will help to provide research-based instruction tailored to your child’s individual needs and learning styles. Educational evaluations help to determine learning differences and disabilities that include dyslexia, dysgraphia and dyscalculia.
NOTE: A medical diagnosis is needed for ADHD and Autism by a medical doctor or psychiatrist.

Evaluation Process
The evaluation process is a series of steps:
1. Contact me. diagdana@myhometownhealth.org
2. I will send you a questionnaire to give me important information about your child.
3. I will contact your child’s teacher(s) and have them fill out questionnaire.
4. If your child is a Grace Prep student, I will observe them in the classroom. If your child attends a public school, I will contact their teacher(s) to gain helpful information.
5. After information has been gathered, it will be determined if your child needs academic and cognitive testing.
6. If testing is recommended, you child will be scheduled for a 3-6 hour series of tests.
7. It will take approximately 2 weeks for results to be reported.
8. A meeting will be scheduled with you (and your older child) to discuss results.
9. Grace Prep Families: A meeting will be scheduled with you (and your older child) and the teachers to discuss a plan and recommendations to help your child. If your child attends a different school, I will send recommendations for your child to your child’s teacher.
